支架式上颌前牵引矫治儿童骨性III类错牙合前后前后软硬组织变化的临床分析研究(1)

[摘要]目的:探讨支架式上颌前牵引矫治器对儿童骨性III类错牙合的矫治效果。方法:采用改良支架式前牵引矫治器对26例儿童骨性III类错牙合患者(7～10岁)进行前牵引治疗。分别在治疗开始(T0)和结束(T1)时拍摄头颅定位侧位片,进行定点测量分析。结果:①硬组织变化: ANB角增加5.26°(P<0.001),Wit's值增加5.26mm(P<0.01),Ptm-A增加2.28mm(P<0.05),Yaxis增加1.25°(P<0.001);SNB角减小0.67°(P>0.05),Go-Me、Co-Gn分别增加0.72mm和1.56mm(P>0.05),SN-PP减小1.24°,PP-MP增加4.2°(P<0.05),颌凸角增加7.07°(P<0.001),下面高、下面高/全面高分别增加2.47(P<0.05)、0.42(P>0.05)。U1-NA角增加4.57°(P<0.001),L1-NB角减小3.21(P<0.05),Ms6-PP距增加0.45mm(P>0.05)。2)软组织变化:面型角增大5.37°(P<0.01),颏唇角减小2.70°(P<0.05),上唇-E线距增大1.51mm(P<0.01),下唇-E线距减小1.25mm(P<0.05)。结论:支架式上颌前牵引矫治器矫治儿童骨性III类错牙合,可产生显著治疗效果:上颌骨前移,上下颌骨的矢状关系改善,凹陷型的畸形容貌显著改善,但高角患者慎用。

Abstract:ObjectiveApplication of fixed frame appliance in maxillary protraction therapy is used to achieve better treatment effects of skeletal Class III malocclusion. Methods26 children of 7～10 year-old with skeletal class III malocclusion were treated with fixed maxillary protraction therapy in this study. Before and after the treatment cephalometric radiographs were taken and analyzed. ResultsAfter maxillary protraction, the anterior crossbite had been corrected and obvious changes of profile can be observed, the maxilla moved foreward, the length increased but followed with PP-SN plane rotates anticlockwise, the mandible moved backward and downward,the ANS-Me,ANS-Me/N-Me increased.For the change of teeth,the upper incisors incline labially and the lower incisors incline lingually with the jaw ......
