近距离霰弹枪击伤致股骨开放性骨折治疗体会(1)
【摘要】 目的 探讨近距离霰弹枪伤致股骨开放性骨折的治疗方法。方法 24例患者伤口初期行彻底清创、切开引流后根据伤口情况行初期、延期或II期闭合伤口，骨折初期采用单边外固定器固定骨折，II期改为交锁髓内钉内固定骨折，修复血管神经损伤。结果 本组24例用此方法均达骨性愈合，患肢功能恢复满意。结论 对近距离霰弹枪击伤致股骨开放性骨折，正确处理伤口后骨折行初期单边外固定，II期改为交锁髓内钉内固定，有效减少感染机会，利于骨折愈合及后期功能锻炼，最大可能恢复患肢功能。【关键词】创伤，枪击；外科手术；骨折固定术
Treatment of open femoral fractures due to shotgun wounds in a short distance

【Abstract】 Objective To discuss the treatment of open femoral fractures due to shotgun wounds in a short distance. Methods 24 shotgun wounds of the femurs were treated by incision and drainage initially, and then were closed in primary, delayed and later phase. The fractures were fixed by unilateral external fixator in primary phase, and then were fixed by internal fixator with interlocking nailings in later phase. Nerves and the vessels were repaired. Results 24 patients participated in this study and all attained osseous healing; the function of the affected extremity was satisfactory. Conclusion Interlocking nailing combined with primary unilateral external fixator may be one of the best options for the management of femoral fractures due to shotgun wounds. ......
