糖尿病白内障患者的临床手术治疗时机与治疗效果分析(1)
[摘要]目的 分析糖尿病白内障患者的临床手术治疗时机与治疗效果。方法 选取2017年4～11月营口鲅鱼圈渤海爱尔眼科医院收治的168例糖尿病白内障患者作为研究对象。根据手术治疗时机的不同，将所有患者分为三组，其中甲组(72例)患者选择空腹血糖<5.9 mmol/L时期进行白内障手术，乙组(66例)患者选择空腹血糖维持在6.0～7.0 mmol/L时期进行白内障手术治疗，丙组(30例)患者选择空腹血糖维持在7.1～8.0 mmol/L时期进行白内障手术治疗。将所有患者治疗后的视力恢复情况与治疗前进行比较，并分析三组患者并发症的发生情况。结果 所有患者的视力均较治疗前得到改善，差异有统计学意义(P<0.05)；丙组患者并发症发生率(86.67%)高于甲、乙两组，差异有统计学意义(P<0.05)。结论 糖尿病白内障患者在进行手术治疗时，应根据其空腹血糖水平合理选择手术时机，可有效改善患者视力，降低并发症发生率。

[Abstract]Objective To analyze the optimal time of clinical surgery for patients with diabetic cataract and its therapeutic effect.Methods From April to November in 2017，168 patients suffering from diabetic cataract treated in the Bayuquan Bohai Aier Eye Hospital，Yingkou City were selected as research objects.According to different time points of the surgeries，they were divided into three groups，group A (n=72) had cataract surgery when fasting blood glucose<5.9 mmol/L)，group B (n=66) had cataract surgery when fasting blood glucose ranging from 6.0 mmol/L to 7.0 mmol/L，and group C (n=30) had cataract surgery when fasting blood glucose maintaining 7.1-8.0 mmol/L.Vision recovery before and after treatment was compared in all patients.The occurrence of complications among three groups was analyzed.Results The vision were all improved after surgeries in all patients compared with that before treatment，with a significant difference (P<0.05).The incidence of complications in group C (86.67%) was higher than that in group A and B，with statistical significance (P<0.05).Conclusion Surgical treatment for diabetic cataract patients should select an appropriate timing based on the fasting blood glucose level，which can effectively improve the vision and reduce the incidence of complications.

近年來，随着社会经济的发展，人们生活水平得到显著提高。饮食结构的改变，使得糖尿病的发病率呈逐年上升的趋势[1]。作为临床十分常见的一种代谢性慢性疾病，糖尿病主要特征为血糖水平异常升高。糖尿病若长时间维持在高水平，可能累及患者各种器官，例如心、肾以及眼睛等，产生各种并发症[2]。其中，糖尿病白内障属于临床常见的一种糖尿病并发症。该疾病好发于青少年糖尿病携带者，且发展较为迅速，一般情况下为双眼共同发病。在发病数周之内，双眼均可出现完全浑浊的情况[3-4]。目前为止，临床尚不完全明确糖尿病白内障的具体发病机制，然而可确定糖尿病长期发展属于发病的主要原因之一。视力下降、晶状体浑浊属于该疾病的主要临床症状，严重影响患者的视力，对其生活质量具有降低作用。本研究选取了我院收治的168例糖尿病白内障患者作为研究对象，对比不同手术时机进行治疗的效果，并获得较为理想的结果，现报道如下。, http://www.100md.com(屈政朋 杨玲)
